Transforming Community-Based Medi-Cal Access: Insights from Dr. Macheo Payne

In an era where systemic change is more crucial than ever, Dr. Macheo Payne, the Director of JSIY systems change at Public Works Alliance, is at the forefront of a transformative movement. His pioneering work focuses on advancing equity for underserved youth and communities by channeling public funding and institutional power to grassroots organizations. This article delves into how Public Works Alliance is reshaping community-based Medi-Cal access and youth empowerment, particularly through Medi-Cal reform, workforce development, and capacity-building initiatives.

Empowerment Through Funding

At the heart of Public Works Alliance’s mission is the belief that power equates to funding. By redirecting financial resources to smaller, community-based organizations, the Alliance enables these entities to better serve youth, especially those at risk of involvement with the criminal legal system. “We see power as funding, money,” Dr. Payne emphasizes. This approach allows these organizations to provide crucial support in non-traditional settings like schools and community centers, shifting the focus from punitive measures to health and wellness.

Medi-Cal Access and Reform

A significant pillar of Public Works Alliance’s strategy is Medi-Cal reform. Dr. Payne highlights the complexities smaller organizations face in navigating Medi-Cal’s intricate funding system. “Medi-Cal is a very complicated funding source,” he notes, “and smaller organizations don’t really have a chance because the expertise required is extensive.” The Alliance addresses this by acting as an intermediary, helping these organizations manage the paperwork and billing processes necessary to access Medi-Cal funds. This initiative is not just about accessing funds but also about ensuring that marginalized communities receive the healthcare support they deserve.

Programmatic Support Across Lifespan

Public Works Alliance’s initiatives span the entire lifespan, beginning with maternal and infant health through their Doula Project. This program supports birthing mothers and babies, ensuring they receive the care needed from the very start of life. Further along the spectrum, the Justice Serving Network (JSN) empowers nonprofits working with justice-involved youth. Dr. Payne describes JSN as “an organizational capacity building initiative,” which provides resources and training to 28 organizations, allowing them to effectively serve their communities.

The Alliance’s EMS Training program is another crucial component, offering career pathways for young adults, particularly those who have been formerly incarcerated. This free, three-month training equips participants with the skills needed to become EMS technicians or paramedics, with the potential for further credentialing. “We purchase boots, outfits, and books,” Dr. Payne shares, emphasizing the comprehensive support provided to ensure participants’ success.

Workforce and Systems Change

Through training and technical assistance, Public Works Alliance helps organizations overcome bureaucratic barriers, particularly in accessing state and federal funding. Dr. Payne underscores the importance of shifting from a punitive model to one focused on health, healing, and opportunity. This systemic change is vital for creating environments where young people can thrive, moving away from the notion of delinquency towards a model of support and empowerment.
